Hah! I received two Esterbook ballpoints in a lot buy last week, both missing refills but with the springs still in the barrels. A little experimentation reveals that Lamy M16 refills will work *very* nicely if you simply pull the plastic plug out of the end of the refill. The tip extends just a wee bit further than would look perfect, but the diameter is dead on and it retracts fully.

I got lucky with the Waterman BP that was also in the lot: it takes a contemporary refill and not one of those $12 Must de Cartier things.
